Direct the medical care and treatment of patients and oversee the care provided by other members of the interdisciplinary care team.
Collaborate with site NP and community-based primary care providers on the shared population.
Identify opportunities to improve the quality, cost, and utilization of care for site participants.
Carry a caseload of patients as the primary care physician and participate in direct medical care at the Diagnostic and Treatment Center.
Lead the medical presence during the start of care events.
Participate in multidisciplinary team activities, including case conferences, hospitalization reports, family, and team meetings.
Develop and participate in the Utilization Review of medical services provided by the Day Treatment Center.
Develop and supervise the provision of ambulatory medical services.
Serve as a liaison with other medical institutions and develop relationships with subspecialists and community physicians for semi-annual care planning.
Facilitate medical education programs for staff, trainees, and students.
Conduct patient assessments, examinations, and treatments in various settings, including the Day Treatment Center and patients' homes.
Serve as a resource for all clinical staff practicing in the Day Treatment Center (nurse practitioners, MAs, RNs).
Provide in-service staff development and clinical teaching of trainees rotating through the medical practice.
Participate in developing preventive health screening, immunization programs, professional community outreach, and quality assurance.
Act within the scope of the individual’s authority to practice.
Meet a standardized set of competencies established by the PACE organization before working independently.
Perform other duties as assigned.
Experience & Skills Required
Residency in Internal Medicine or Family Practice is required.
Fellowship, diploma, or experience in geriatrics and palliative care is preferred.
Board certification in Internal Medicine or Family Practice is required.
No malpractice or loss of license at the Day Treatment Center.
Excellent verbal and written communication skills.
Minimum of one (1) year of experience working with a frail or elderly population, or appropriate training from the PACE organization upon hiring if less than one year of experience.
Current active unrestricted license to practice medicine in New York State.
Be legally authorized to practice in the state.
Be medically cleared for communicable diseases and have up-to-date immunizations before engaging in direct participant contact.
Must have a Medicaid Provider ID and a Medicare Provider ID in good standing.
