Physical Therapist (PT) - Home Health Contract

Opportunity Description

About the Position:

We are seeking a dedicated and compassionate Clinic Physical Therapist to join our team at our Ridgewood, Queens location.  As a Clinic Physical Therapist, you will play a vital role in providing high-quality care to our participants, especially the geriatric population.

Job Duties

Assessment and Treatment: Conduct comprehensive physical therapy assessments and develop individualized treatment plans for participants based on their specific needs.

Therapeutic Interventions: Administer physical therapy treatments, exercises, and modalities to improve participants' physical function, mobility, and overall well-being.

Education: Educate participants and their families about their condition, treatment options, and home exercise programs to facilitate their recovery.

Documentation: Maintain accurate and up-to-date participant records in accordance with regulatory and organizational standards using electronic medical records systems like Care Compass.

Collaboration: Collaborate with a multidisciplinary team of healthcare professionals to provide holistic care to participants, ensuring their physical and emotional well-being.

PACE Regulations: Stay informed and compliant with PACE regulations to ensure the highest quality of care for our participants.

Home Visits: Travel to participant's homes when necessary to deliver therapy services

Experience & Skills Required

Education: Successful completion of a Physical Therapy program from an APTA accredited university or equivalent.

Experience: Minimum of one (1) year of experience as a Physical Therapist. Experience working with the geriatric population is preferred.

License Registration: New York State licensure as a Registered Physical Therapist.

Language: Bi-lingual proficiency, depending on the region and participant demographics, is advantageous.

Physical Requirements:

Ability to lift and transfer participants.

Ability to bend, kneel, reach, stretch, stand, and lift up to 50 pounds.

Other Requirements:

Ability to establish rapport with patients and staff.

Ability to travel to participant’s homes, nursing homes, and sites/clinics.

Knowledge of PACE (Program of All-Inclusive Care for the Elderly) regulations.

Knowledge of Care Compass and other Electronic Medical Records (EMRs) preferred.
