Opportunity Description

HumanEdge Allied Health Staffing is seeking a compassionate and skilled Home Health Registered Nurse for a full time 13-week contract. In this home health contract, the hired RN will be responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Local and travel Registered Nurses are encouraged to apply. Experience with electronic charting using HomeCare HomeBase is required.

Location: Houston, TX
Territory: Covers branch service area - specfic coverage details will be discussed in branch-level interview
Setting: Home Health
Contract Details: 13 weeks, 40 hours per week
Productivity: 30 units (weighted by visit type) for 40 hours
EMR System: HomeCare HomeBase

Job Duties

Conduct initial and ongoing patient assessments to identify patient needs
Dvelop individualized care plans
Provide care to patients in their homes, including wound care, medication management, and disease management
Educate patients and families on disease management, medication management, and preventative care measures
Communicate and coordinate with interdisciplinary healthcare team members
Document and maintain accurate records using HomeCare HomeBase
Respond promptly to patient needs and emergency situations
Make appropriate referrals as needed

Experience & Skills Required

Degree in Registered Nursing from an accredeited college or university program
Active RN license in the state of practice - Compact licensure accepted in applicable states
Experience with electronic charting using HomeCare HomeBase
At least 6 months of experience in home health care
Excellent clinical skills
Strong communication and interpersonal skills
Ability to work independently and efficiently manage a patient caseload
Ability to prioritize and re-arrange tasks and schedules as required

Other Information

All HumanEdge compensation packages are custom made to meet your goals and needs, and will vary for each individual. Packages for Home Health RNs typically range from $1200 - $2000 or more per week. Travel stipends are available for those who qualify.
