Physical Therapist (PT) - Home Health Contract

Opportunity Description

ALBUQUERQUE, NM - HumanEdge has partnered with a local home health provider to hire a PT for a 13-week contract assignment. This home health provider is a long-time, trusted client of HumanEdge, and this branch is very welcoming and open to contract staff. Local and travel PTs are welcome to apply.

40 hours per week
Mon-Fri main schedule
Some weekend coverage may be required, based on patient needs
Occasional on-call may be required, based on patient needs

Job Duties

Prioritize workload so as to meet productivity standards and applicable deadlines
Communicate with branch staff on a regular basis about patient status and potential issues
Maintain a positive and respectful attitude and demeanor with all staff and patients
Provide initial and ongoing assessment to determine the level of functioning, including OASIS Assessment
Select and modify evaluation tools, and evaluate patients to determine physical and psychosocial functioning
Plan and implement treatment program
Document all services in Homecare HomeBase

Experience & Skills Required

Home Health: 2+ years
PT: 2+ years
HCHB: 1+ year

Other Information

All HumanEdge compensation packages are individually created and may vary between providers. Average weekly pay can range from $1,800 - $3,000 or more per week. Untaxed stipends are available for those who qualify.
