Opportunity Description

Our Client facility is a prominent healthcare institution located in Wayne, New Jersey, dedicated to delivering exceptional patient care and fostering a supportive work environment. They are currently seeking a dedicated and experienced Social Worker to join their dynamic team.

Job Duties

Conduct initial intake assessments and weekly progress evaluations related to discharge activities.

 

Collaborate with a multidisciplinary team to ensure comprehensive patient care.

 

Uphold high standards of patient care and satisfaction.

 

Utilize strong organizational and time management skills to meet discharge goals.

 

Demonstrate effective interpersonal and communication skills.

Experience & Skills Required

Bachelor’s Degree in Social Work (MSW or LMSW certification preferred).

Licensed Social Worker (LSW) in the state of New Jersey.

Minimum of 1 year of social work experience, preferably in an acute care setting.

Strong computer skills necessary, with proficiency in relevant software.
