Opportunity Description

HumanEdge is partnered with an amazing organization to recruit strong Hospice RNs for a weekend day position.  It is a 24 hour work schedule.  

 

 

 

Job Duties
  • Admits patients to service for Hospice
  • Evaluate hospice patients as needed
  • Perform assessment of patient and family, through observation, interviews, and analysis of records.
  • Analyze physical, emotional psychosocial, environmental and spiritual needs.
  • Begin development of a comprehensive care plan.
  • Completes paperwork required by agency, government, and third-party systems
Experience & Skills Required
  • Graduation from an accredited school of nursing; BSN preferred
  • CT RN License
  • A minimum of two years of nursing experience
  • Home Health Care, Hospice/End of life care experience preferred
  • Certified Hospice and Palliative Care Nurse (CHPN) preferred
