Job Title: Nurse Practitioner Supervisor

Schedule: 8:00 AM – 5:00 PM (40 hours per week)
The Nurse Practitioner Supervisor (NPS) will collaborate with contracted community primary care physicians to ensure effective oversight and coordination of participant care within the Interdisciplinary Team (IDT). This role focuses on addressing the functional, clinical, and psychosocial needs of participants, emphasizing independence and wellness.
Supervise and manage Nurse Practitioners at assigned locations and assist with overseeing other clinical staff.
Conduct annual wellness assessments for all assigned participants, with increased frequency for those at higher risk.
Provide urgent care services and facilitate same-day access for participants.
Assess participants post-hospitalization within five days, ensuring comprehensive care transitions.
Review and maintain accurate documentation of participants' medical histories and current conditions.
Participate in clinical documentation improvement activities to capture the burden of illness effectively.
Co-manage high-risk participants with community physicians and coordinate care plans.
Attend IDT meetings and represent the community physician when necessary.
Educate participants and families on disease processes, medication adherence, and self-management.
Conduct home visits as needed and oversee the implementation of care plans.
Provide emergency coverage for primary care providers as required and participate in on-call rotation.
Comply with all HIPAA regulations and maintain the security of protected health information.
Experience & Skills Required
Minimum of two years in clinical nursing practice in home care or care management.
Minimum of one year of experience working with frail or elderly populations, with training provided if experience is lacking.
Current Nurse Practitioner license in NY.
DEA certification required.
AANP or ANCC Board Certification required.
