Field RN Position: Clinical Assessment Manager – Care Delivery
Job Summary: This field-based role involves conducting clinical assessments, onboarding, and reassessments for PACE participants. Responsibilities include in-home visits, UAS-NY completion, medication review, and post-hospital discharge evaluations, ensuring compliance with NYS Department of Health regulations.
Conduct in-home UAS-NY assessments to determine care eligibility.
Work closely with intake and care delivery teams, documenting clinical data in the electronic system.
Promote PACE benefits to potential participants and referral sources.
Travel within various boroughs and be available for weekend work as needed.
Experience & Skills Required
BS in Nursing (preferred), RN license required.
3-5 years of managed care experience.
Experience working with frail or elderly populations.
Hours: 8:30 AM – 5:30 PM, 40 hrs/week
