The Nurse Practitioner Supervisor (NPS) – Mobile Program leads and coordinates participant care in collaboration with PACE Physicians, community primary care physicians, and the Interdisciplinary Team. The role focuses on meeting participants’ clinical, functional, and psychosocial needs, emphasizing independence and wellness. The NPS serves as a liaison between participants, community physicians, nursing homes, and plan staff to ensure comprehensive care plans are developed and executed.
Supervise Nurse Practitioners and oversee clinical staff at assigned locations (nursing homes and PACE sites).
Perform annual wellness assessments and urgent care services.
Manage high-risk participants, coordinating care with primary physicians and interdisciplinary teams.
Facilitate post-hospital follow-up care, medication reconciliation, and care plan adjustments.
Conduct home visits and support care plan implementation in the community.
Engage in complex care management, educate participants and staff, and ensure compliance with HIPAA regulations.
Participate in on-call rotation and provide emergency coverage as needed.
Experience & Skills Required
Experience: Minimum 2+ years of clinical nursing, home care, or care management experience.
Skills: Excellent communication, analytical, and project management skills.
License: Must have a Medicaid and Medicare Provider ID.
