Social Worker - Case Manager

Opportunity Description
HumanEdge Health has an immediate need for a a Social Worker ( LCSW or LBSW) for a travel contract in Pittsfield, MA 

Job Duties:
  • Perform comprehensive admission and discharge risk assessment 
  • Utilization management
  • Discharge planning
Requirements
  • Current knowledge of CMS, Appeal Right, and DPH regulations
  • Current Knowledge of Kepro QIO ( appeals)
  • Knowledge of Joint Commission, DPH and HealthGrades 
  • Knowledge of Medicaid, Medicare, and 3rd Party commercial Payers and Reimbursement
  • Masters in Social Work Required 
  • LSCW required, LICSW preferred 
  • 3 years of clinical experience  - acute care or rehab LTC
